Departments
Federal Regulations state that schools shall make substitutions in foods for students who are considered disabled and whose disability restricts their diet. Crestwood School District will accommodate special diets according to the following guidelines.
Food Allergies
Food substitutions will be made as required by the child's physician for allergies covered under the Americans with Disabilities Act. A medical statement form is required as well as advance notification of the days the child will be eating lunch at school. All Medical Statement Forms must be updated at or before the beginning of each school year.
The Food Allergen Labeling and Consumer Protection Act of 2004 (FALCPA) requires food manufacturers to clearly list all allergens or their proteins which may be contained in foods. This includes the 8 major food allergens: milk, eggs, Fish, (egg., bass, flounder, cod), Crustacean shellfish (e.g., crab, lobster, shrimp), tree nuts (.g., almonds, walnuts, pecans), peanuts, wheat, and soybeans. Manufacturers are NOT required to place advisory statements such as "processed in a facility" or "processed on equipment" These notices are voluntary.
As with any medical condition, if your child has an allergy, please contact the school office for more information. The health and safety of your child is very important to us, and we have procedures in place to help keep your child safe while in our care. The district nurse will work with you to create a plan for your child at school.
For students that have a disability or medical need for a food substitution, please complete the form below, this form must be completed by a physician. Once the form has been completed, return it to the food service office (you can ask your student's school to send it to us). Once we have received your form we will contact you to discuss procedures and menu options for your student.
